Sports Physicals

The end of the school year is fast approaching but let’s get ready for the next one. Did you know that your annual Well Child Check is also a sports physical? If your child has had their Well Child Check within the last year we can fill out their sports physical forms. You can call our office to check the date of their last Well Child Check or schedule their next check-up.

Please get the sports physical forms here, fill out the medical history portion, and bring them with you. We’ll fill out the rest.

Flu SHOTS

Flu shots are usually available in our clinic from late August until the following spring. During the season we usually have flu mist for children 3 and up, and flu shot for children 6 months old and up. If you’d like your child vaccinated, please call for an appointment during the season.

Office hours on 9/23/25-9/24/2025

Due to staff training, our office will have limited hours on Tuesday, September 23, and Wednesday, September 24, 2025.

• On September 23, we will be seeing patients from 8:30 AM to 11:40 AM.

• On September 24, we will be available from 1:00 PM to 4:30 PM.

PARKING & ACCESS

Our clinic is on the campus of Renown Regional Medical Center.

We now have reserve parking for our patients. Reserved parking is located in the second street parking garage on the 2nd floor.
